How to Set Up a Home Desk That Actually Helps Your Back

A decision framework for fixing back and neck discomfort at a home desk, grounded in OSHA, HSE and NHS ergonomics guidance. Start with height and the chair, not the most expensive gadget.

Most people with a sore back at home try to buy their way out of it, and they buy the wrong thing first. A standing desk arrives, the discomfort stays, and the chair that was actually causing it never gets touched. The evidence on home-office ergonomics is less about owning the right equipment than about hitting a few measurable targets: where your elbows sit, where your eyes land on the screen, and whether your lower back is supported. This is a framework for getting there in a sensible order, using guidance from workplace safety and health authorities rather than a shopping list.

It is written for the remote or hybrid worker who already has discomfort and a normal budget, not for someone building a showroom.

Start with one number: elbow height

Almost everything downstream depends on getting your working height right, and the reference point is your elbows, not your desk.

The UK Health and Safety Executive describes the target plainly: sit so your forearms are roughly level, with the keyboard just below elbow height and your shoulders relaxed, high enough that you are not shrugging. OSHA frames the same neutral posture as elbows kept close to the body and bent between about 90 and 120 degrees, with the forearms approximately parallel to the floor. NHS guidance from hospital ergonomics teams puts a similar figure on it, setting the seat so the elbows sit roughly level with, or just above, the desk surface.

The practical move is to set the chair first so your elbows meet that height, and only then ask what your feet and your desk are doing. If raising the chair lifts your feet off the floor, you need a footrest so your thighs stay supported and your knees stay near hip level. A stack of books works. If the desk is now too high and forces your shoulders up, the desk is the problem to solve, not the chair. This is why height is the first decision: it tells you which of the other pieces actually needs money spent on it.

The chair does more for your back than the desk

If the discomfort is in your lower back, the chair is almost always where the fix lives, and the two things that matter most are lumbar support and seat depth.

Lumbar support means the backrest follows the inward curve of your lower spine instead of leaving it to slump. Hospital ergonomics guidance is direct about the mechanism: push your bottom to the back of the seat so the pelvis cannot tip backwards, because that backward tip is what strains the lumbar spine. The backrest should support the natural curves of the spine rather than flatten them. You do not need a famous chair to get this. A rolled towel or a small cushion in the lower-back gap reproduces most of the effect on a chair you already own, and the HSE explicitly treats a cushion as a legitimate way to add back support or raise your seated height.

Seat depth is the quieter culprit. If the seat pan is too long, the front edge presses into the back of your knees and you cannot use the backrest properly. The HSE target is a gap of roughly two to three centimetres between the front of the seat and the back of your knee. Many chairs have a sliding seat pan to adjust this; if yours does not and the seat is too deep, a firm cushion behind your back shortens the effective depth. Armrests, where they exist, should sit at about elbow height with the shoulders relaxed, so they carry some arm weight rather than forcing your shoulders up.

Monitor height is the cheapest fix for neck pain

Neck and upper-back discomfort usually traces back to a screen that is too low, which pulls the head forward and down for hours. This is the single most common home-office mistake and one of the cheapest to correct.

The shared guidance across authorities is consistent. The HSE says the top of the screen should be roughly level with your eyes, about an arm's length away. OSHA adds a useful refinement: the center of the screen normally wants to sit around 15 to 20 degrees below horizontal eye level, with a preferred viewing distance somewhere between about 50 and 100 centimetres depending on the screen and your eyes. The simple test is to sit back in your set-up chair, close your eyes, look ahead, and open them; your gaze should land on the top third of the screen, not the middle or the bottom.

A laptop cannot meet this on its own, because fixing the screen height lifts the keyboard out of reach and fixing the keyboard drops the screen too low. The HSE answer is to separate the two: raise the laptop on a stand or riser so the screen comes up to eye level, then plug in a separate keyboard and mouse at elbow height. A laptop stand plus a basic external keyboard is one of the highest-value purchases in this whole exercise, and it is a fraction of the cost of a desk. A monitor arm does the same job for an external display and frees desk space, but a stable stack of books under the monitor reaches the right height for nothing. The height target matters far more than the hardware that achieves it.

Keyboard and mouse: keep the wrists neutral

Once height is right, wrist posture is mostly about placement. The keyboard should sit just below elbow height and centred in front of you, with the mouse in line with the same elbow so you are not reaching out sideways. The HSE stresses keeping the screen and keyboard directly in front so you are not twisting your back to face them. Leave enough room in front of the keyboard to rest your hands and forearms between bursts of typing. Wrists should stay straight rather than bent up or angled out; if they are bending, the cause is usually the chair height or the desk height, which loops you back to the first decision rather than to a wrist gadget.

What the evidence actually says about standing desks

Here is where the spending often goes wrong, because the sit-stand desk is sold as a health upgrade and the evidence is more modest than the marketing.

A Cochrane systematic review looked at workplace interventions to reduce sitting and found that sit-stand desks do reduce sitting time, by somewhere between about 30 minutes and two hours over a working day. That part is real. But the review was blunt about the rest: the potential health benefits are very uncertain, and the quality of the evidence was low, drawn from small and often poorly designed studies. In short, a sit-stand desk can help you sit less, but the claim that it fixes back pain or transforms your health is not well supported.

Two practical conclusions follow. First, the benefit is in alternating, not in standing all day; standing rigidly for hours brings its own fatigue and discomfort and is not the goal. The win is breaking up long static periods by changing posture. Second, you do not need a powered desk to get that win. The most consistent advice across the same authorities is simply to move and change position regularly, with hospital guidance suggesting nobody should sit unbroken for much more than an hour. A timer that prompts you to stand, stretch, or walk for a couple of minutes delivers the part of the standing-desk promise that the evidence actually backs, at no cost. If you find standing genuinely helps you concentrate, or you have specific lower-back discomfort that eases when upright, a sit-stand desk is a reasonable buy, just bought for the right, honest reason.

Cheap fixes first, big purchases last

Put in order, the framework spends money where it does the most good and leaves the showroom items for last.

The free or near-free fixes come first: set chair height to your elbows, add a cushion or rolled towel for lumbar support, raise the monitor to eye level on whatever is sturdy, pull the keyboard and mouse in close, and set a timer to break up sitting. These address the most common sources of discomfort directly. The mid-cost additions are the ones that solve a problem you have actually identified, most often a laptop stand with an external keyboard and mouse, or a footrest if raising the chair lifted your feet. The big purchases, a properly adjustable chair or a sit-stand desk, are worth it when a real, specific limitation remains after the cheap fixes, for example a chair with no lumbar support or seat-depth adjustment that a cushion cannot rescue.

The mistake is reversing that order. Buy the targets, not the products: elbows level, eyes meeting the top of the screen, lower back supported, and regular movement. Hit those, and a modest set-up will help your back more than an expensive one that misses them.
